Located in Southern California, Mental Health Transitions is the only stand-alone transitional living licensed by the State of California in Orange County. THE GAP BETWEEN TREATMENT & INDEPENDENT LIVING
Good mental health treatment exists. However, there are limited options upon completing treatment. How do you practice coping skills when challenges arise without trained staff there to support you? Who supports you with medication management, appointments, and refills? Families are often put in this role causing more complications in their relational dynamics.
One in five Americans lives with a mental illness; one in twenty-five has a severe mental illness, and many of these people never get the help they need. These statistics also do not consider the many hospitalizations and programs many individuals have been through, they go home, and the cycle continues. Our program cultivates a sense of safety, community, and sustainable recovery for people recovering from mental illness.
OUR APPROACH TO IMPROVING MENTAL HEALTH OUTCOMES
We created Mental Health Transitions as a step-down option to enable people to integrate skills learned in residential programs or other stabilization experiences. We have found that many individuals need more support than what’s typically offered in an outpatient level of care. We have also seen how many people lack a safe, supportive living environment.
We are bridging the gap between residential or psychiatric care and independent living; our supportive services include therapy, case management, life skills, 24-hour staffing, and more. We develop a plan of action unique to each individual. Some clients are ready to get a job, go to school, or volunteer. Others attend local outpatient programs for more structured support at this stage. We will work with you to create the path that serves your journey.
